The legacy Application Insights exporter is not reliably recognized by the AI Gateway monitoring experience. This change configures the native OTLP exporter after Application Insights provisions its managed endpoints and DCR.

• Bicep: Enable AzureMonitorWorkspaceIngestionMode, remove the legacy inline exporter, and expose the Application Insights, Log Analytics workspace, and Gateway principal IDs.
• Postprovision hooks: Register Microsoft.Monitor, wait for the managed DCR and ingestion endpoints, and assign Monitoring Metrics Publisher at the DCR scope using a direct ARM PUT. Configure the OpenTelemetry exporter with the resource ID, signal endpoints, and managed identity audience:

• Migration and docs: Delete an existing exporter before recreating it when its immutable kind is not OpenTelemetry. Document the managed-resource-group deny-assignment constraint and preview behavior where exporter GET responses may omit tracesEndpoint.
• Coverage: Extend the AI Gateway model-registration test with assertions for ingestion mode, outputs, and hook configuration.
