Relay reconnects could execute completed tool calls again after their IDs were evicted from the deduplication cache, repeating side effects.

Keep verified event IDs until the server transport closes, with a separate bounded cache for encrypted envelopes. This also blocks rewrapped duplicates; memory grows with unique verified events during the transport’s lifetime.

Added regression tests and documentation. Verified 665 passing tests (5 skipped) and 5,100 replayed encrypted requests with zero duplicate executions; lint, type checking, build and package checks pass.

Fixes #105.
