Restructure the Ultra and Swarm prompts around explicit role, scope, workflow, constraint, and output sections. - Replace planner and reviewer LS access with command tools - Restrict planner commands to inspection and verification - Keep reviewer commands limited to read-only review work - Raise the Swarm capacity to 128 agents and five levels - Extend admission and prompt contracts for the new limits
