Adds a predict, review, and apply workflow for model-generated required phrases. Every prediction requires an explicit decision before it can be applied.
The command saves review decisions, shows the exact rule diff, supports phrase editing, rejects stale review records, validates the complete apply batch before mutation, and writes each changed rule once.
Validation: 62 focused tests, Ruff,
git diff --check, and a smoke prediction with the final hardened model.
