Adds explicit initial-failure stage for prospective nonbinding repair profiles. Requires confirmed failure and verified local evidence, omits unverified observation flags, and permits abstention. Default evidence-reviewed policy and every mandatory repair gate remain unchanged. Diagnostic delivery is measured separately from completed evidence review and causal ranking.
Verification: eight targeted tests; 831 full Python tests passed in 37.595s; graph rebuilt. Unknown IDs, missing failure/discriminator, late acknowledgment and failed repair remain rejected. No live trial, historical rescoring or release.
