Description
Part 1 of #100. Adds a pino logger configured with LOG_LEVEL, plus per-request logging in app.ts. Documents LOG_LEVEL in .env.example and the environment variables guide, and adds unit tests for the logger.
Converting the existing console.* calls in auth.ts and the controllers is a mechanical follow-up PR (part 2), kept separate to keep this one small. Other console.* calls (e.g. in the clients) are untouched by both parts.
