Pass DuckDB label as a default application_name - #617

This PR sets the [application_name](https://www.postgresql.org/docs/current/libpq-connect.html#LIBPQ-CONNECT-APPLICATION-NAME) connection parameter to `DuckDB` when it is not specified by user in connection string or in `APPLICATION_NAME` or `URI` options of the `SECRET`, that used to `ATTACH` the Postgres database. Fixes: duckdb#613

This PR updates Postgres and SQLite bringing in the following fixes: Postgres: - duckdb/duckdb-postgres#617 - duckdb/duckdb-postgres#620 - duckdb/duckdb-postgres#621 SQLite: - duckdb/duckdb-sqlite#256 - duckdb/duckdb-sqlite#258 Additonally SQLite patch `0003-check-file-access-of-local-databases` is absorbed.
