Fixes #434

On Postgres, fetched_at is a TIMESTAMP without zone. It was written in local time but read back as UTC, so east of UTC metadata_ttl expired hours late. It is now stored in UTC. Entries cached before the update expire late one last time, then are stored in UTC. #428 does the same for artifacts.
