Lowercase the namespace and name of vscode-extension PURLs in Parse, New, MakePURLString and BuildPURLString. The spec makes both case-insensitive, and packageurl-go v0.1.7 leaves them as given.

MakePURL, MakePURLString and BuildPURLString now accept VS Code's native publisher.name identifier, so MS-Python.Python becomes pkg:vscode-extension/ms-python/python. Identifiers without a publisher return an empty result. Versions, platform and repository_url are unchanged, so Open VSX packages keep their registry.

Fixes #41

Lowercase the publisher and extension name, which the spec makes case-insensitive, and split VS Code's publisher.name identifiers.
