docs: ship README banner in repo and add conduct contact - #3

The README banner pointed at the live site's Open Graph image, which 404s until the site is deployed. Commit the image under .github/assets and link it relatively so it always renders on GitHub. Fill in the Code of Conduct reporting address. Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>

What does this change?
https://ephemera.kerrshift.com/opengraph-image, which doesn't exist until the site is deployed, so it rendered as a broken image on GitHub. The image now lives at.github/assets/ephemera.pngand is linked relatively, so it always renders.CODE_OF_CONDUCT.md(it was a placeholder).Checklist
