Grok 6653958 + ee8f88b (base integration/0.5.48). Closes T185's two gaps:

  • Fork: a Claude SessionStart with source fork returns before bindSession, so the parent sessions row stays and the session gets only the mailbox-context fallback line.
  • Busy: an identity held live by another session prints the held-by-live reason with no takeover suggestion.
    Fixtures in test/hooks.test.ts failed first (4a38a42). hooks 30/30, watch-midturn 6/6, project-mailbox 26/26, hook-connection 20/20, tsc clean, no dist drift. CHANGELOG Unreleased only. Ships in integration/0.5.48 (integration/0.5.48 (T698): T696, T697, T583, T677, T187, T667, T185 #371).

A Codex rollout mtime and a live Grok session row keep a stale mbx call from reading as idle. A dead or quiet harness still falls back to the mbx time.
A running turn or a host with no session note API keeps the bounded resume note and reports one fallback. A fresh session and a quiet tool result still show the note. Mail is not read or acked.

Fork must leave the parent session row and print the MCP fallback.
A live holder must emit the held-by-live line with no takeover suggestion.
