fix(responses): report spend-ledger storage refusals as local 503s - #6890
Conversation
A SpendLedgerFileRefusedError (for example an extra hard link from a sync daemon) was caught as a transport failure, returned as 502 Provider unreachable, attributed upstream, and charged to provider and account health. Map it, including nested retry causes, to 503 server_error spend_ledger_storage_unavailable before transport classification on passthrough, translated, compact and runTurn paths; log it as a local refusal and skip health penalties. The guard is unchanged. Refs #6314.

Map storage failures from translated continuations
When the terminal guard starts a translated continuation, fetchContinuation() in adapter-continuation.ts performs the same spend-ledger reservation/reporting as the initial exchange, but its catch blocks convert every thrown SpendLedgerFileRefusedError into an unstructured Provider continuation failed event. Because this new mapping only covers prepareAdapterExchange, a guard that trips between the initial send and its continuation is still reported as a generic streamed 502 and attributed upstream rather than carrying spend_ledger_storage_unavailable and local-refusal; apply the mapper in the continuation catches and emit the structured terminal error there too.

Preserve storage refusals through sidecar dispatch
For requests routed through the image or web-search sidecars, executeResponsesSidecars passes the same adapter send budget into the loop, so a ledger safety check can fail during an adapter-owned send; however, the loop catch blocks normalize that exception to LoopError(502, "Provider unreachable: ...") (and image runTurn producer failures are similarly wrapped), discarding the typed cause before this mapper can see it. Such sidecar requests therefore retain the old provider-attributed 502 behavior; recognize the refusal before generic sidecar transport wrapping and return or emit the structured local 503.

Keep the zero-send marker off dispatched requests.
If a retry reaches the provider before the ledger refuses a later send, this call still sets logCtx.localTerminalReason. tests/responses/responses-send-budget-errors.test.ts covers that one-send state at Lines 247–253. The field means that the request was answered locally without an upstream send, and addFinalRequestLog passes its presence to finalizedUsage as locallyAnswered. Record the synthetic refusal and error code without setting localTerminalReason when a send has already dispatched. Preserve local-refusal attribution through a marker that does not claim a zero-send answer.

Summary
A spend-ledger storage refusal is now reported as a local 503 with its own code, instead of "Provider unreachable" 502.
When the journal fails a safety check (for example
journal: extra-hard-link, which a sync daemon can cause inside a synced Documents folder, as in #6314), the guard correctly refuses to write. Every dispatch path then caught that refusal as a transport failure: the client got HTTP 502 "Provider unreachable: Spend-ledger storage could not be opened safely", the request log attributed it to the upstream, and passthrough recorded provider and account health failures for a problem that never left the machine. Now a shared mapper (
src/server/responses/spend-storage-error.ts) recognizesSpendLedgerFileRefusedError, including when it is nested inside a retry wrapper's cause chain. Passthrough, translated dispatch, compact and runTurn check it before transport classification:type: "server_error",code: "spend_ledger_storage_unavailable", with the existing sanitized message (file role, failed check, synced-folder guidance; no private path).formatErrorResponseandclassifyErrorpreserve this code, so a generic upstream 503 is stillserver_is_overloaded. A response that already started streaming carries the code in its terminal error.local-refusal.Internal retries do not change, because 502 and 503 are both already in the transient-status and combo-hop sets. Clients now see a 503 with a specific code where they used to see a 502.
The troubleshooting page and
structure/transports/responses-spend.md/responses.mddescribe the new response. The diagnostics and Google Drive detection named in the issue thread had already landed in #6398 and #6846; this PR covers the remaining misclassification. The extra link itself comes from outside OpenCodex.Refs #6314
