Summary

Carry #7117 by @luvs01 so requests cannot accumulate unlimited consumers of a shared OAuth refresh after disconnecting. Pending refresh resolution now has an independent process-local ceiling of 256 consumers alongside the existing 32-flight ceiling. Fresh cached credentials bypass admission; capacity refusal returns HTTP 503 with Retry-After: 1 without quarantining accounts, and cancellation wins with 499. Already-committed runTurn SSE reports a server error.

The original contribution is preserved by cherry-picking b722d746f10dafb7eeea2aabcf8b94aec6a6a5db as 0cb3868b03. Follow-ups keep stale owners registered through definitive-result cleanup and durable retirement even with spare capacity, recheck single-flight ownership when concurrent replacements resume, and observe both request and option cancellation signals during initial auth refusal and runTurn recovery. Deferred durable retirement logs once per flight without account or credential identity. Owning runtime/failover contracts and public troubleshooting guidance are updated. No GUI or dependency changes.

Verified against origin/dev at 7640a942b35044c974e534b84d5a8049f4a8ae9a: the consumer gate and capacity mapping were absent. The source PR had no submitted reviews or inline review comments when inspected; CodeRabbit skipped the draft. It links a fork PR, with no closing issue references. The original PR is left for the coordinator to manage.

Authentication/OAuth changes require explicit maintainer security review under MAINTAINERS.md; that review remains pending. This PR is implementation and test evidence, not security approval. The limit bounds unresolved consumer count, not retained bytes, provider duration, or multiple processes. An abort-ignoring refresh retains finite capacity until it settles.

Verification

All runtime checks used temporary HOME, OPENCODEX_HOME, CODEX_HOME, CLAUDE_CONFIG_DIR and XDG directories, plus the repository's test preload; synthetic credentials only. isolate below denotes that environment wrapper. The test runner was a scratch-local Bun 1.4.3 matching package.json; no global runtime or real account configuration was changed.

  • bun install — passed before implementation; no tracked lockfile changes.
  • isolate bun test --isolate --timeout 30000 tests/oauth/oauth-refresh-stale-capacity.test.ts tests/oauth/oauth-refresh.test.ts tests/oauth/oauth-refresh-generic-lock.test.ts tests/oauth/oauth-refresh-lock-multiprocess.test.ts tests/responses/responses-oauth-refresh-capacity.test.ts tests/responses/responses-devin-401-replay.test.ts tests/oauth/adapter-event-oauth-failover.test.ts tests/oauth/generic-oauth-failover.test.ts tests/providers/kiro/kiro-leased-responses.test.ts tests/codex-integration/active-turn-disconnect.test.ts tests/codex-integration/active-registry-admission.test.ts tests/test-layout.test.ts tests/test-layout-tooling.test.ts tests/ci-workflows/file-size-ratchet.test.ts — 322 pass / 0 fail / 3,051 assertions, 14 files.
  • isolate bun run typecheck — passed.
  • isolate bun run structure:check — passed.
  • isolate bun run privacy:scan — passed.
  • (cd docs-site && bun install --frozen-lockfile && isolate bun run build) — passed; 577 pages and 81,203 internal links checked.
  • git diff --check origin/dev — passed.

Red controls on the carried contributor tree: the independent-signal cancellation cases yielded 1 pass / 1 fail (expected 499, received 503); the unsaturated late-rejection/retirement-IO cases yielded 0 pass / 2 fail. The final run above covers the corrections. Hosted Codex identified a runTurn request-only cancellation gap: three new cases failed before the correction (including committed SSE), then passed with both client signals linked and cleaned up. CodeRabbit identified silent retirement failure; a once-per-flight identity-free diagnostic and bounded-log regression address it. The diagnostic red control yielded 1 pass / 1 fail before the log existed. An initial run on the shell's Bun 1.4.0 had 316 passes and one tooling failure because that guard requires Bun 1.4.3; using the repository-pinned runtime resolved it without changing the guard. Counts from overlapping runs are not added.

The maintainer prohibits heavy/full local suites on this shared host. The full suite, prepush, and whole tests directory were therefore NOT RUN locally. Current implementation head: 6fba175cac2ff1a55e5c2d92ea85d9e5f2a5c291. Broad Ubuntu and cross-platform validation belongs to hosted CI; exact-head PR checks and a subsequent workflow_dispatch with lane=all will be inspected separately. Live-provider refresh, an end-to-end resource-exhaustion benchmark, and operational sizing of the 256-consumer limit were not established.

Checklist

  • Scope stays focused and avoids unrelated cleanup.
  • Docs or release notes were updated when needed.
  • Security-sensitive changes were reviewed for secrets, auth, and unsafe defaults. Privacy scan passes; explicit maintainer security review is pending.

Summary by CodeRabbit

  • Bug Fixes
    • Improved handling of busy OAuth token refreshes. When refresh capacity is reached, requests receive a retryable 503 response with a one-second retry delay instead of being treated as an authentication failure.
    • Requests with fresh credentials can continue, and cancelled requests retain their cancellation response.
  • Documentation
    • Updated the sign-in troubleshooting guide with guidance for the refresh-capacity error, including retry timing and expected behavior.

Bound unresolved OAuth refresh consumers while preserving stale-flight ownership evidence across refused replacement admission. Report retryable capacity separately from authentication failures, and preserve cancellation priority and lease-release assertions.

(cherry picked from commit b722d74)
Keep stale registry ownership through definitive-result cleanup and durable retirement regardless of spare consumer capacity. Recheck the installed flight when concurrent replacements resume.

📝 Walkthrough

OAuth refresh resolution now limits pending consumers to 256 and retains stale flights until settlement and cleanup. Responses paths map capacity refusals to retryable errors, with cancellation taking precedence. Tests and documentation cover these behaviors.

Changes: OAuth refresh capacity

Layer / File(s) Summary
Refresh admission and stale-flight retirement
src/oauth/index.ts, tests/oauth/*, structure/runtime.md, docs-site/src/content/docs/troubleshooting/codex-cannot-sign-in.md
OAuth refresh resolution limits pending consumers, including callers joining an existing flight. Stale flights remain registered until settlement and cleanup. Tests cover capacity, retries, fresh credentials, and stale-flight outcomes. Documentation describes the capacity limit and refusal behavior.
Responses capacity refusals
src/server/responses/*, tests/responses/responses-oauth-refresh-capacity.test.ts, tests/providers/kiro/kiro-leased-responses.test.ts, scripts/test-layout/layout.json, tests/fixtures/test-layout-expected.json, structure/transports/responses-failover.md
Responses paths return retryable 503 responses for capacity refusals before commitment, while cancellation returns 499. The runTurn path handles refusals in streaming and buffered requests. Tests cover transport paths, cancellation, and response state; documentation and test-layout mappings include the new coverage.

Pure move of retireSettledOAuthRefreshFlight, the refresh-waiter gate and OAuthRefreshFlightEvidence out of src/oauth/index.ts, so this branch merged with current dev stays under the 2000-line file-size ratchet. The registry map is passed explicitly; behavior is unchanged.

Maintainer integration into dev (MAINTAINERS.md dev maintainer-integration policy; batch integration authorized by @lidge-jun on 2026-10-15).

  • Head: 75a8da42f8d03d6afecdb50761dc7ff92f21d97d (tree bb1180396eef7b7fc4691968f72a9642d904b2fe) = this branch merged with origin/dev 8ac597aee2. Integration note: extra commit moves retireSettledOAuthRefreshFlight, the refresh-waiter gate and OAuthRefreshFlightEvidence to src/oauth/refresh-flight-retirement.ts (pure move) to keep src/oauth/index.ts under the 2000-line ratchet together with the rest of the batch.
  • Exact-head evidence: GitHub-hosted Actions for this account have not been assigning runners since 2026-10-15 13:30 UTC, so by owner decision the evidence is a full bun run test (Bun 1.4.3 test runner, root and GUI dependencies installed, GUI built) of this exact tree on a maintainer host (Linux x86_64): exit 1, 7 min, 28 failing test(s), each also failing in the same host's baseline run of dev 60bf4c7 (environment-only): Codex shim install readiness > a refused install exits unsuccessfully and preserves its re, Codex shim install readiness > keeps install advisory when the Codex config cannot be read, Codex shim install readiness > the install command surfaces readiness warnings without lea, GUI pairing client > sends one bodyless origin-bound capability and redacts rejected bodie, Node launcher PATH fallback activation > broken bundle reaches validated PATH Bun, Node launcher PATH fallback activation > inspection bundle reaches validated PATH Bun, Node launcher PATH fallback activation > missing bundle reaches validated PATH Bun, bundledBunPath / durableBunPath > PATH-selected process provenance survives durable select, ….
  • Local gates on the same tree: bun run typecheck, file-size ratchet, test-layout and structure:check.
  • Security review: independent agent security review PASS at 6fba175 (consumer admission held through settlement; no credential disclosure). The extra move commit is behavior-neutral.
  • Hosted CI checks for this head may still be queued; they are not claimed as evidence.
