Summary

  • Clear account-scoped failure samples, degraded state and pin warnings on account deletion/recreation or physical login replacement behind __main__, preserving sibling accounts.
  • Carry pre-dispatch main identity/credential proof or stored Pool credential generation through terminal, transport, compaction and sidecar outcomes. Retain Pool sample generations and revalidate them on insertion/read, including cross-process replacement and A-to-B-to-A transitions.
  • Distinguish failed credential snapshots from normal absent/empty stores. Read/parse failure preserves samples, degradation, recovery timer, retained generation and pin-warning history; successful empty snapshots still retire old generations. Existing boolean liveness callers keep their compatibility behavior. No separate per-candidate snapshot reuse optimization is included.
  • Reject arrays and scalar top-level JSON before store row normalization; malformed shapes preserve existing window evidence, while missing files and {} remain valid empty snapshots. Individual row normalization is unchanged.
  • Preserve existing health, quarantine and probe ownership. Add focused regression coverage and update the owning architecture contract within its line budget.

Verification

The candidate used Bun 1.4.3, repository-root preload/home isolation and the official shared test lock. Publication does not rerun tests or CI.

  • Latest top-level-shape baseline: bun test tests/codex-integration/codex-failure-window.test.ts --test-name-pattern 'wrong-shaped top-level snapshot' — 1 pass / 5 fail / 29 filtered, 22 assertions. [], false, 0, empty/nonempty strings erased the window; null already preserved it. These were meaningful assertion failures, not timeouts.

  • Latest focused correction/controls: bun test tests/codex-integration/codex-failure-window.test.ts --test-name-pattern 'wrong-shaped top-level snapshot|valid .* snapshot still retires|temporary .* snapshot failure preserves' — 12 pass / 23 filtered / 0 fail, 72 assertions, 14.41 seconds. Covers all six top-level shapes, read/parse failure preservation, normal {} and ENOENT retirement. Existing suites were not expanded or repeated.

  • Latest candidate type/structure/privacy/diff checks and official scoped 15-file ratchet — PASS. The following 56/9 and earlier-stage results are preserved as validation of their respective stages.

  • Current-head CI is complete for 49309523d283719760d6c498b55f8fa4dd2fff0a: 23 successful check runs / 9 skipped / 0 failed / 0 pending, plus CodeRabbit SUCCESS at this readiness decision. Cross-platform CI run 39123580184 completed successfully on this head, including Ubuntu shards 1–4, gates and final ci. Skipped jobs and the unrun full local suite are not passes. Current dev remains 8640c2f30edbd2fbb6947ed5c25e2793f5ae72a6.

  • Snapshot-failure baseline before the correction: bun test tests/codex-integration/codex-failure-window.test.ts --test-name-pattern 'snapshot failure preserves degraded' — 0 pass / 2 fail / 25 filtered, 6 assertions. Both fixture read failure and malformed on-disk JSON erased previously degraded state. These were deterministic assertion failures, not timeouts.

  • Previous snapshot-failure focused validation: bun test tests/codex-integration/codex-failure-window.test.ts tests/codex-integration/codex-account-store-quota-isolation.test.ts tests/codex-integration/codex-model-denial-evidence.test.ts — 56 pass / 0 fail, 172 assertions, 18.07 seconds. Covers preservation during failure and recovery, unchanged absent/empty retirement, tagged and untagged insertion, prior identity/generation fences, the 512-sample single-snapshot bound, quota-publication isolation and existing denial behavior.

  • Previous snapshot-failure loader compatibility: bun test tests/codex-integration/codex-account-store.test.ts --test-name-pattern 'round-trip|legacy flat|malformed credential store|tokenful tombstone|listCodexAccountIds|generation CAS|not found|new saves write|malformed optional' — 9 pass / 61 filtered / 0 fail, 25 assertions. Covers legacy records, malformed-store handling, tombstones, missing accounts and generation CAS.

  • bun node_modules/typescript/bin/tsc --noEmit, bun scripts/structure-ssot.ts, bun scripts/privacy-scan.ts, and git diff --check — PASS. The architecture document stays within 600 lines.

  • Official evaluate / loadBaseline / countLines / isOffender ratchet on exactly the 15 changed files — PASS, zero offenders. No repository-wide ratchet pass is claimed; the earlier sparse-checkout ENOENT was not a pass.

  • Publication readback: all 15 remote Git blobs match the reviewed final candidate. Current commit 49309523d283719760d6c498b55f8fa4dd2fff0a has 230e8040bc7eb0fffe8d16e1a0d727dfb6f27647 as its sole parent, changes exactly two files, and has a valid GitHub signature.

  • Historical previous-head CI for 230e8040bc7eb0fffe8d16e1a0d727dfb6f27647: 24 successful check runs / 9 skipped / 0 failed / 0 pending at the readiness decision, plus CodeRabbit SUCCESS. Cross-platform CI run 39121509675 completed successfully on this head, including Ubuntu shards 1–4, gates and final ci. Skipped jobs and the unrun full local suite are not claimed as passes. Current dev remains 8640c2f30edbd2fbb6947ed5c25e2793f5ae72a6.

  • bun run test:changed and the full suite were not run locally due to shared-host resource contention. The documented focused-regression/typecheck resource exception is used; no full local-suite pass is claimed. GUI checks/screenshots: N/A, no GUI files changed.

Earlier-stage evidence is retained separately: main-login baseline 1 fail, followed by 54 pass and expanded 73 pass / 1 existing skip / 0 fail; cross-process baseline 1 fail, followed by 78 pass / 1 existing skip / 0 fail (382 assertions). Initial cleanup was 43 pass / 5 fail: four default five-second timeouts and one pin fixture error. Re-pinning corrected the fixture; quieter filtered validation at the same budget was 6 pass / 0 fail, supporting environment sensitivity without establishing a unique timeout cause. The existing skip was the regular native SSE reporter case and is not counted as a pass.

Historical CI on the previous heads completed successfully: cross-process head CI and dispatch-fence head CI. These are historical results, distinct from current-head CI.

All four review threads are resolved. CodeRabbit explicitly confirmed the top-level isObject guard, unchanged row normalization, and valid empty/absence behavior. No unresolved actionable finding was present at this readiness decision; any newly triggered reviews remain separate. Maintainer sponsorship and required security review remain before-merge responsibilities under CONTRIBUTING.md and MAINTAINERS.md.

Checklist

  • Scope stays focused and avoids unrelated cleanup.
  • Docs or release notes were updated when needed.
  • Security-sensitive changes were reviewed for secrets, auth, and unsafe defaults.

Explicit readiness decision: this exact head is ready for maintainer review using the documented focused-regression/typecheck resource exception and successful exact-head CI. This is not merge authorization; maintainer sponsorship/security review remains required before merge.

  • Required local validation passed with its scope documented.
  • Branch is on the latest dev commit.
  • All correct Codex and CodeRabbit findings have been fixed.
  • Ready for maintainer review.

Summary by CodeRabbit

  • Bug Fixes
    • Removed accounts and changed main-account logins no longer inherit stale failure or degraded-state warnings.
    • Clearing one account’s failure state leaves other accounts unaffected, and recreated pinned accounts can show warnings again when appropriate.
    • Outcomes from replaced credentials or identities no longer affect the current account’s failure window or create misleading routing warnings.
    • Temporary account-store read or parsing failures preserve existing failure-window state, while valid empty stores retire old state.

Forget account-scoped failure samples and pinned degradation warnings during deletion and physical main-account replacement. Preserve sibling account windows and cover both lifecycle paths.

📝 Walkthrough

Codex failure-window outcomes now carry dispatch-time credential and identity provenance. The recorder rejects stale or unowned outcomes and preserves window state when account-store snapshots fail. Account runtime-state cleanup also clears the account’s failure window.

Changes: Failure-Window Integrity

Layer / File(s) Summary
Validate generations and failure-window snapshots
src/codex/account-store.ts, src/codex/routing/failure-window.ts, tests/codex-integration/codex-failure-window.test.ts
Account-store checks now report snapshot status. Failure-window logic rejects stale generations and preserves state when a snapshot read or parse fails. Integration tests cover replacements, races, absent or empty snapshots, and recovery timing.
Capture and propagate outcome provenance
src/codex/auth-context.ts, src/codex/routing/cooldown-math.ts, src/codex/routing.ts, src/providers/openai-sidecar.ts, src/server/responses/compact.ts, src/server/responses/core-codex-account.ts, src/server/responses/passthrough-delivery.ts, src/server/responses/passthrough-dispatch.ts, tests/codex-integration/codex-failure-window.test.ts
Main-pool contexts capture dispatch proof when the quota writer is live. Routing and response paths pass failure-window metadata with outcomes, including terminal, retry, sidecar, and passthrough outcomes. Tests cover stale callbacks and credential-replacement races.
Clear failure-window state on account changes
src/codex/account-lifecycle.ts, structure/providers/openai-accounts.md, tests/codex-integration/codex-account-delete-atomicity.test.ts, tests/codex-integration/codex-main-rotation.test.ts
purgeCodexAccountRuntimeState clears failure-window state by account ID. Documentation describes its ownership and provenance rules. Tests verify deletion cleanup, sibling isolation, warning reset, and main-account identity replacement.

Retain each stored Pool sample's dispatch credential generation and filter it against one current store snapshot per insertion or lookup. Reset retired-generation degradation, recovery timers and pin warnings without clearing a replacement on stale arrival. Cover both unlocked-read boundaries, derived state and the 512-sample snapshot bound.

🎯 Functional Correctness | 🟡 Minor | ⚡ Quick win

Do not publish an empty roster for failed snapshots.

Non-ENOENT read, parse, and top-level-shape failures notify notifyCodexStoreLoaded({}) before returning status: "failed". The quota consumer treats that notification as an authoritative empty snapshot, replaces the current Codex roster, and removes still-valid quota evidence. This can affect quota decisions during a transient failure.

Scoped review at 49309523d2: the patch now distinguishes failed/unknown store snapshots from valid empty/removal, retains generation tags on samples, captures dispatch provenance for terminal callbacks and purges the main alias on physical replacement. Those are the relevant boundaries, rather than merely clearing a map during delete. Keep failure-window versus other health/lease semantics distinct, and retain same-ID replacement, late old callbacks, context reuse, malformed store, recovery timer and pinned-warning regressions in the head receipt. Current Ubuntu CI succeeded. I have not completed full credential/dispatch review or run local/native tests, so this is not an approval of the whole account lifecycle change.
