…and lifecycle paths (#7388) * fix(responses): recover explicitly rejected opaque state on cold history * fix(chat): check reasoning replay budgets before copying Preflight escaped-JSON size and reserve translator budget before materializing native Chat reasoning replay. Preserve existing budget errors, caller objects and retry/key-rotation ownership; retain synthetic regressions and update accounting contracts. * docs(chat): clarify replay size checks and budget reservations Distinguish the fixed serialized-request cap from the full final-size transient translator reservation. Document changed-replay ordering and finally release without changing production or test behavior. * fix(responses): cap aggregate reasoning archive storage * fix(responses): preserve archive scan errors during cleanup * fix(responses): normalize forward base URLs with a linear scan Co-authored-by: Devin AI <158243242+devin-ai-integration[bot]@users.noreply.github.com> * fix(empty-completion): separate a discarded attempt's reasoning from the retry's The guard holds pre-content events across an empty-completion retry and releases them with the retry's output. With nothing between the two attempts, an unsigned reasoning delta from the retry is appended to the discarded attempt's still-open reasoning item, so the client sees one reasoning item whose text runs both attempts together ("discardedretry"). Each retry point now queues an assistant_boundary after the held events (only when something is held), which the bridge and the direct encoders already treat as "close the open items". Co-Authored-By: Claude Code <noreply@anthropic.com> * fix(lifecycle): end a tracked stream when its turn controller is aborted A drain past its deadline calls abortAndReleaseAllTurns, which aborts every controller bound to an admitted turn. trackStreamLifetime only stopped on a consumer cancel, and several callers (the adapter bridge, run-turn execution, client-encoder delivery, policy refusals, local SSE) pass a fresh AbortController that nothing else listens to. For those turns the abort was a no-op: the stream kept relaying upstream output and its source stayed open. trackStreamLifetime now listens for the controller's abort and, once tracking has started, finishes the turn, cancels the source and errors the stream. The listener is attached after registerTurn so the lease's own abort handling still runs first, and a controller registerTurn aborted synchronously (a settled lease) keeps the previous behaviour. Co-Authored-By: Claude Code <noreply@anthropic.com> * fix(lifecycle): stop the tracked stream before onDone on abort A throwing onDone callback must not leave the source open. Co-Authored-By: Claude Code <noreply@anthropic.com> * fix(responses): sanitize reasoning archive refusals on the WebSocket transport The HTTP boundary already maps typed archive failures to a sanitized 503 with a stable code. The WebSocket pre-response catch sent them as a raw 502 proxy_error frame that carried the archive path. Apply the same mapping there. --------- Co-authored-by: Ingwannu <ingwannu@users.noreply.github.com> Co-authored-by: Epinephrine <luvs01@hanmail.net> Co-authored-by: Devin AI <158243242+devin-ai-integration[bot]@users.noreply.github.com> Co-authored-by: Vadym O <bolein95@gmail.com> Co-authored-by: Claude Code <noreply@anthropic.com>
…e accounting and Kiro refusal hop (#7389) * fix(responses): run the dispatch override on the Codex WS branch providerFetch applied `dispatchOverride` only on the HTTP path. The WebSocket branch called codexWsUpstreamFetch directly, so a key-auth WebSocket send (api.openai.com with upstreamWebsocket) never reached commitKeyAttemptSend or rebindPhysicalSend: its attempt row reported sendCount 0 for a frame that left. The WS send now runs through the same override; the override hands a marked WS executor its send directly instead of HTTP connection policy, and the HTTP fallback inside it skips the override so one logical send is counted once. Co-Authored-By: Claude Code <noreply@anthropic.com> * fix(responses): send a dispatch-time reselection over HTTP, not the WS WS eligibility (egress, pacing, URL) is checked against the provider the WS executor was built for. The dispatch override can replace route.provider before it calls the executor; that replacement was never checked, so it could ride a WebSocket its own config rules out (e.g. an explicit proxy). The WS executor now records the provider it was admitted for and its HTTP fallback; when the override serves a different provider it sends through that fallback under the replacement's own egress route. Co-Authored-By: Claude Code <noreply@anthropic.com> * fix(responses): rotate a pooled API key on a native passthrough 429 The native Responses passthrough returns before the adapter recovery loop that rotates pooled keys, so a key-pool provider's 429 was returned to the client while the same refusal rotates on the adapter and chat-native wires. Rotate to the next key before output, inside the shared send budget. Distributed round-robin selection keeps its cool-without-replay behaviour. Co-Authored-By: Claude Code <noreply@anthropic.com> * fix(usage): seal a credential-rotation attempt with the status it answered When an account rotation splits the attempt row, the sealed attempt with no recorded status fell back to 401 for key-401, 429 for any *-429 kind, and 502 for everything else. An `oauth-account-403` rotation was therefore logged as a 502 server error, and `oauth-401` / `oauth-account-402` likewise, although the recovery kind names the status the account actually returned. Map those kinds to 401, 403 and 402 too. A status already recorded on the attempt still wins. Co-Authored-By: Claude Code <noreply@anthropic.com> * fix(usage): a leg refused before any send does not make the row unreported A multi-attempt row takes its usage status from aggregateAttemptUsage, which let any usage-less attempt mark the whole row "unreported". A combo leg the gateway refused locally (input admission, a local cooldown) never reached upstream and carries no usage, so a combo that then served the turn on its next leg with fully reported usage was still logged as unreported. The status now ignores usage-less attempts that never sent or were answered locally -- the same "spent nothing" rule summary.ts already prices by. An attempt that was sent and reported nothing still leaves the row unreported. The existing "folds partial and unsupported" test built its unreported 503 without a send; it now notes the send, which is the case it describes. Co-Authored-By: Claude Code <noreply@anthropic.com> * fix(usage): sum cache reads across attempts that report different fields aggregateAttemptUsage summed `cachedInputTokens` and `cacheReadInputTokens` independently. Anthropic usage sets both; an OpenAI-shaped attempt sets only `cachedInputTokens`. Row-level readers take `cacheReadInputTokens ?? cachedInputTokens` (request-log, management/shared.ts), so a row aggregating an Anthropic leg (60 read) and an OpenAI-shaped leg (40 cached) reported 60 cache reads instead of 100. Cost is priced per attempt and is not affected. Once any attempt reports `cacheReadInputTokens`, the aggregate's read field now counts each attempt's `cacheReadInputTokens ?? cachedInputTokens`. Rows whose attempts never report the read field are unchanged. Co-Authored-By: Claude Code <noreply@anthropic.com> * fix(messages-native): answer a spent Anthropic pool as a usage limit with its reset #6991 made the bridge lane answer a 429 as "All Anthropic OAuth accounts have reached their usage limit; the first one resets at <time>" once the last pool account refuses on spent quota and rotation finds no replacement. The managed native Messages lane rotates through the same pool but, when it runs out, relays the upstream rate_limit_error ("try again later") unchanged, so Claude Code on that lane never learns the window is spent or when it reopens. After rotation ends on a 429, the native lane now asks the same anthropicPoolUsageLimitAfterRefusal the bridge uses and, when it holds, answers through the lane's existing AnthropicAccountCooldownError mapping (429 rate_limit_error, the usage-limit message, Retry-After). Co-Authored-By: Claude Code <noreply@anthropic.com> * test(messages-native): pin request-id retention on the pooled usage-limit answer Co-Authored-By: Claude Code <noreply@anthropic.com> * fix(kiro): rotate on a 400 its validator calls improperly formed Kiro answers `400 ValidationException: Improperly formed request` when the identity a request was routed under cannot serve it. classifyKiroRefusal returned `other`, so the rotation loop stopped: the account stayed selected, no other login was tried, and inside a combo the 400 ended the chain even though a later target could serve the turn. Classify it as the account-scoped kind `profile` (400 only, and both the vendor exception name and the "improperly formed request" wording are required, so a genuinely bad payload or a caller echoing one token stays terminal). Rotation and the existing Kiro cooldown apply unchanged. comboFailureDecision treats the same shape, also inside a transport-wrapped frame, as a hop, before the generic invalid_request_error stop. * fix(kiro): fence the improperly-formed hop to Kiro targets; parse refusal fields * fix(responses): keep the passthrough key-429 rotation to one call site Fold the budget check into passthrough-key-rotation.ts and import both passthrough credential moves through it, so passthrough-dispatch.ts grows by one line (1997) and stays under the 2000-line ratchet threshold. Co-authored-by: vadymhimself <11277453+vadymhimself@users.noreply.github.com> * test(combos): move the Kiro improperly-formed hop case to a sibling file combos.test.ts would reach 2000 lines (NEW_OVERSIZED). The case moves byte for byte into combos-kiro-profile-hop.test.ts, registered in layout.json and test-layout-expected.json. Co-authored-by: hedging8563 <45883076+hedging8563@users.noreply.github.com> * fix(combos): fence the Kiro profile hop by the adapter the failed leg resolved A kiro provider can route a model through another wire via modelAdapters; the configured adapter would still have granted the hop to a non-Kiro 400. Pass the child leg's resolved providerAdapter instead; an unresolved leg grants nothing. Adds an e2e regression that fails on the configured-adapter fence. …locks, and scope Direct fallback to the locked main (#7393) * fix(codex): renew opted-in pool credit evidence on the existing sweep * test(codex): assert renewal performs no native-main access * fix(codex): limit Direct hard-lock fallback to the locked account Use the original route's credential ownership and observed-main caller check before applying Direct hard-lock fallback. Preserve binding, missing-bearer and disabled-route behavior; document and cover the boundary. * fix(responses): break hard-lock caller-check import cycle Move the caller-check factory into core-auth, which owns the credential-domain helpers, and update its request-preparation import. Keep the function body and routing/authentication policy unchanged. * fix(codex): revalidate hard-lock fallback targets before request rewriting * fix(codex): clear failure windows when account identities are removed Forget account-scoped failure samples and pinned degradation warnings during deletion and physical main-account replacement. Preserve sibling account windows and cover both lifecycle paths. * fix(codex): fence late failure-window samples by dispatch identity Carry the main dispatch proof and stored Pool credential generation into terminal outcomes. Reject stale success and transient samples after identity or credential replacement, while preserving existing health and probe handling. Cover main identity ABA, same-ID Pool recreation, context reuse, and valid-current controls. * fix(codex): revalidate tagged failure samples after credential replacement Retain each stored Pool sample's dispatch credential generation and filter it against one current store snapshot per insertion or lookup. Reset retired-generation degradation, recovery timers and pin warnings without clearing a replacement on stale arrival. Cover both unlocked-read boundaries, derived state and the 512-sample snapshot bound. * fix(codex): preserve failure windows when credential snapshots fail Distinguish failed reads or parsing from valid absent/empty credential stores. Preserve retained samples and derived failure-window state while evidence is unknown, keeping existing boolean liveness callers compatible. Cover fixture read/parse failures, recovery and empty-store retirement without adding snapshot reuse optimization. * fix(codex): reject non-object credential snapshot shapes Require a non-null, non-array object before normalizing store rows. Preserve failure-window evidence for invalid top-level arrays or scalars while keeping missing files and empty objects valid. Cover six top-level shapes without changing row normalization. * refactor(responses): keep passthrough-dispatch within its line budget * fix(codex): keep a paused main caller out of Pool fallback Carries the paused-main regression from #7290 and the production guard it proves is needed, plus a per-generation credit-renewal pacing test requested at intake. Co-authored-by: Hylouis233 <88263959+Hylouis233@users.noreply.github.com> * fix(codex): recheck a predicted-reset main hard lock with metadata (#7312) A predicted reset now arms the capped WHAM recovery schedule instead of suppressing it, so an early reset redeemed outside opencodex is learned from verified metadata. The schedule never runs past the predicted reset, skips paused and needs-reauth mains, and a predicted reset still never lifts the lock by itself. * test(codex): await the async hard-lock fallback override in #7340 cases #7328 made applyMainHardLockFallbackOverride async for the Direct caller check; the #7340 revalidation cases were written against the synchronous form and asserted on a Promise once both landed. Co-authored-by: luvs01 <27862058+luvs01@users.noreply.github.com> * fix(codex): recheck Direct hard-lock ownership synchronously before rewriting The async caller check now resolves to a synchronous ownership proof that is evaluated after the last await, so a main identity or bearer change during the policy-binding wait cannot reroute the original caller onto the fallback. Co-authored-by: luvs01 <27862058+luvs01@users.noreply.github.com> * fix(codex): keep predicted-reset hold rechecks paced across transitions (#7312) Track the first hold per identity/credential separately from the backoff reservation so transient 401/403 retries are not re-armed, arm a hold that appears during token preparation instead of reading immediately, pull the due time in when the predicted reset moves earlier or passes, and keep hold reads at least one base interval apart. * fix(codex): honor a revised reset that elapses between hold sweeps (#7312) Recovery scheduling now reads the latest blocking reset from current evidence even after it has passed, so a reset revised earlier and expired before the next sweep releases the old reservation. It remains scheduling evidence only; admission still requires verified metadata. --------- Co-authored-by: Ingwannu <ingwannu@users.noreply.github.com> Co-authored-by: Epinephrine <luvs01@hanmail.net> Co-authored-by: Hylouis233 <88263959+Hylouis233@users.noreply.github.com> Co-authored-by: luvs01 <27862058+luvs01@users.noreply.github.com>
…d audio adapter fixes for 2.90 (#7394) * fix(google): keep Gemini progress-history splitting linear Replace growing-prefix boundary checks with fixed-size suffix checks and one-pass whitespace tracking. Preserve progress-history semantics with bounded-work and parser/adapter regressions, and update the provider contract. * fix(openai-chat): reconcile identified MiMo tool-call echoes * test(openai-chat): cover streamed invalid and paired identified tool echoes * fix(google): keep Claude tool results contiguous before images * feat(google): honor showThinkingSummary on Vertex and direct Gemini Cloud Code Assist Gemini requests opt into generationConfig.thinkingConfig.includeThoughts and classify returned thought: true parts as thinking_delta summaries when provider.showThinkingSummary is enabled, while Vertex and direct Gemini only forwarded thinkingLevel and left Google streaming silent for the entire reasoning phase (triggering 200s response-byte inactivity timeouts on long reasoning turns). Honor provider.showThinkingSummary on thinking-eligible Vertex and direct Gemini requests as well, subject to the same per-request hideThinkingSummary gate, non-image guard, and Gemini model-family check. Unopted providers, unladdered Vertex models, image models, and explicit summary:"none" requests remain unchanged. * fix(google): require a non-empty reasoning ladder for thinking eligibility * test(google): cover stream parity and adapter reuse for showThinkingSummary * docs(test): add JSDoc comments to thought-summary test helpers * fix(mistral): enable vision sidecar for GLM 5.3 * Declare GLM 5.3 native text input for vision sidecar enrollment. * Cover catalog publication, image preprocessing, and operator overrides. * Document image attachment support and client catalog refresh. * docs(mistral): clarify catalog sync and client restart * Name the sync command and full client restart steps. * Explain restart effects and verify image attachments after relaunch. * test(mistral): assert GLM vision override in catalog * feat(opencode-go): update model catalog with Gemini 4 Argon, GPT-6 Luna, DeepSeek V4.1, and preview models * feat: translate inline audio to chat completions Parse Codex input_audio data URLs into a typed IR part, send WAV/MP3 as Chat input_audio parts, and promote tool-result audio into a follow-up user message. Refuse unsupported or remote audio at the adapter boundary and charge decoded bytes during input admission. * test: cover translated chat audio forwarding * docs: describe inline audio on the chat wire * fix: tighten translated audio media guards Refuse the Chat-shaped audio part in Responses input, keep developer audio out of the translated Chat role path while preserving developer documents, and add Cursor and Ollama fallback coverage. * docs: define translated audio media contract Update the adapter authority, Chat compatibility, inbound boundary, and byte-accounting contracts for supported inline WAV/MP3 audio, tool-result promotion, refusal positions, native passthrough boundaries, and decoded-byte admission. * test: register chat audio layout coverage * docs: clarify orphan audio and raw base64 Include orphan-tool-result audio in the promotion contract and document raw base64 with an explicit WAV/MP3 format. * Keep DeepSeek image replays below the provider body limit * fix: scope image preparation and retain rebuild upload pacing * fix(opencode-go): leave GPT-6 Luna tiered pricing unset GPT-6 Luna bills by prompt-length tier, so one unconditional rate would misdescribe it. Keep its cost unknown like DeepSeek V4.1 Flash (peak/off-peak) and regenerate the metadata from the source snapshot. Co-authored-by: yeongjunyoo <47925973+yeongjunyoo@users.noreply.github.com> * docs(structure): move the Mistral GLM vision-sidecar note into the images data plane providers-and-adapters.md has a 600-line budget; the carried note fits the vision-sidecar contract in data-planes/images.md instead. Co-authored-by: adaurban <31841049+adaurban@users.noreply.github.com> * fix(chat): carry standalone MiMo freeform recovery Port shirruy's standalone bare-block recovery from #7260 (6da6d18), with Markdown-aware mid-line holding scoped to the synthetic-call path. Keep identified-wrapper reconciliation and its ambiguity rules separate. Retain only bounded detached candidates across streaming prose release; reject nested headers and disable recovery when an enclosing block becomes ambiguous. Preserve fenced/inline examples and invalidate candidates when structured calls arrive. Add buffered/streamed regressions and sync the structure contracts without growing openai-chat.ts past 822 lines. Co-authored-by: shirruy <63145023+shirruy@users.noreply.github.com> * docs(chat): describe bounded MiMo bare-call recovery Document the declared-freeform-only recovery gate, literal examples, identified-wrapper separation and streaming/argument retention bounds. Co-authored-by: shirruy <63145023+shirruy@users.noreply.github.com> * fix(deepseek): fence queued image uploads on persistence consent A queued upload rechecked only the credential selection, so revoking deepseekImageFiles or reloading configuration while it waited still sent the image to /files. Capture the configuration generation with the provider snapshot and refuse the physical upload when it or the model's opt-in changed. Co-authored-by: fontanierh <14199823+fontanierh@users.noreply.github.com> * fix(opencode-go): never undercut the declared DeepSeek V4.1 Flash window The registry declares 1,048,576 tokens for opencode-go/deepseek-v4.1-flash; the snapshot row carried the rounded 1,000,000 figure and failed the existing never-undercut invariant. Co-authored-by: yeongjunyoo <47925973+yeongjunyoo@users.noreply.github.com> * test(responses): inventory the DeepSeek image-file transport owner Co-authored-by: fontanierh <14199823+fontanierh@users.noreply.github.com> * fix(openai-chat): preserve opaque context and bound bare-call scanning Carry Markdown context through the shared serialized-content buffer on all providers. Advance newline-header lookahead monotonically and stop standalone candidate traversal at its count bound, discarding any saved batch on overflow. …ate paths and shim token handling (B5) (#7397) * fix(oauth): allow dismissing a settled failed login flow (carry #7379) Co-authored-by: Ingwannu <186453546+Ingwannu@users.noreply.github.com> * fix(providers): preserve explicit API-key mode when keys are unavailable (carry #7304) Co-authored-by: luvs01 <27862058+luvs01@users.noreply.github.com> * fix(dsh): bound account profile response reads (carry #7333) Co-authored-by: luvs01 <27862058+luvs01@users.noreply.github.com> * test(dsh): drain server fixtures before removing homes (carry #7305) Co-authored-by: luvs01 <27862058+luvs01@users.noreply.github.com> * fix(claude-cli): keep local errors out of subscription cooldown (carry #7343) Co-authored-by: luvs01 <27862058+luvs01@users.noreply.github.com> * fix(claude): bound Desktop library reads and preflight writes (carry #7301) Co-authored-by: luvs01 <27862058+luvs01@users.noreply.github.com> * fix(service): stop scheduler wrappers before Windows reinstall (carry #7313) Co-authored-by: luvs01 <27862058+luvs01@users.noreply.github.com> * fix(startup): require RunAtLoad before Desktop relaunch (carry #7277) Co-authored-by: luvs01 <27862058+luvs01@users.noreply.github.com> * fix(codex): strip OpenSSL startup variables from updater child env (carry #7342) Co-authored-by: luvs01 <27862058+luvs01@users.noreply.github.com> * fix(codex): keep the service token out of bypassed shim launches All three Codex shims loaded the service API token into OPENCODEX_API_AUTH_TOKEN before checking OCX_SHIM_BYPASS, so a bypassed native passthrough launch handed the proxy data-plane token to the real Codex and its descendants. The POSIX, CMD and PowerShell shims now skip the token load under bypass; the Unix revision marker moves to 4 so installed wrappers are refreshed. loadServiceTokenFromFile reads the inherited OCX_API_TOKEN_FILE through a no-follow, nonblocking descriptor, accepts only a regular file of at most 4 KiB and returns only a single-line printable token. * test(claude): wait for a real ctime transition before requiring rejection Re-applying the same utimes moves ctime only after the kernel clock ticks, so the real ctime case from #7301 often observed no change and the reader correctly returned {}. Repeat the call until ctime has advanced (bounded), assert that precondition, then require desktop_config_not_regular. Co-authored-by: luvs01 <27862058+luvs01@users.noreply.github.com> * fix(google-antigravity): admit a probed project_id fallback in account import (carry #7385) An Antigravity account whose Cloud Code Assist project is not auto-discovered was rejected with missing_project even when the operator knows its project. A Cockpit record may now carry a bounded project_id (Google Cloud project grammar, 6-30 chars, exact match). Discovery keeps precedence; the fallback is admitted only after the account quota probe succeeds with that account own access token against that project. Probe refusal, probe errors and cancellation write nothing. No shared default project. Co-authored-by: yobushka <51312852+yobushka@users.noreply.github.com> * fix(quota): name a closed reason when an OAuth account quota probe fails Anthropic per-account quota rows reported quotaUnavailable with no reason, and Antigravity reported account_unavailable for any credential-side throw. The Anthropic usage read now reports a closed code for an HTTP status class, an unusable body or a thrown dispatch, and a probe that cannot obtain a credential reports account_unavailable without dispatching. Antigravity distinguishes a credential refresh deadline (timeout). Codes reuse the existing closed QUOTA_FAILURE_CODES set, carry no upstream text and never change account health. * test(claude): match the symlink-resolved rollback target in the picker restore case The atomic writer renames onto the symlink-resolved target, so on macOS (/var -> /private/var) the rollback rename never matched the fixture path and the restore-failure injection did not fire. Compare both spellings. Co-authored-by: luvs01 <27862058+luvs01@users.noreply.github.com> * fix(google-antigravity): cancel the import project probe with the import request probeAntigravityUsageQuota accepts an optional caller signal that cancels every request of the probe and stops before the models fallback; the import verifier forwards the import signal and never admits after cancellation. * fix(codex): upgrade installed Windows shims that load the token before bypass CMD and PowerShell wrappers now carry a Windows revision marker. A marked wrapper without it (installed before the bypass-token fix) is obsolete: installCodexShim and autoRestoreCodexShim rewrite it in place over the same backing launcher instead of reporting it healthy or already installed, and diagnostics report it unhealthy. On Windows the service token reader also binds the opened descriptor to the path by identity, so a link swapped in after lstat is refused. * test(codex): mark the in-place Windows diagnostics fixture with the current revision * fix(codex): refresh obsolete Windows shims without clobbering a concurrent replacement The obsolete-wrapper refresh truncated whatever occupied the wrapper path. It now moves the observed wrapper aside and proves it is the probed file, stages the current wrapper beside it, and publishes with a hard link that refuses an occupied path. A concurrent replacement is restored or kept untouched and the refresh reports deferred. * docs(structure): record the Windows shim refresh transaction * fix(codex): create the Windows shim refresh stage exclusively and resume partial upgrades The refresh stage is now created with O_CREAT|O_EXCL (and O_NOFOLLOW on POSIX) under an independent random name, and publication, withdrawal and cleanup are bound to the identity of the file this refresh created, so a planted link is never written through. Auto-restore resumes a Windows revision-only sibling mix (current plus obsolete wrappers) instead of deferring forever after one transient publish failure. * fix(codex): bind Windows shim refresh rollback and recovery to observed identities Publication keeps a linked entry only if it is the created stage, and withdraws a link to a swapped stage (file or symlink) while leaving independent replacements. Restoring the moved wrapper requires the aside to still be the moved file. Automatic revision recovery now refreshes only the admitted obsolete wrappers and first re-verifies every current sibling, so a replacement during the stability wait is never adopted. * fix(codex): never displace a Windows shim where its replacement cannot be published The refresh proves hard-link support in the wrapper directory with a throwaway pair before moving the wrapper aside, and if a restore link still fails it falls back to an identity-checked rename onto the empty path, so the launcher never stays missing. --------- Co-authored-by: Ingwannu <186453546+Ingwannu@users.noreply.github.com> Co-authored-by: luvs01 <27862058+luvs01@users.noreply.github.com> Co-authored-by: yobushka <51312852+yobushka@users.noreply.github.com>
… limit DeepSeek upload roles (#7396) * fix(openai-chat): never synthesize a bare MiMo call from a cut stream A MiMo stream that closed after a complete bare block but before finish_reason or [DONE] reached the EOF flush with user-facing output, and standalone recovery emitted an executable call that structured calls on the same path would have refused as possible truncation. Recover only after a terminal signal; otherwise the block stays visible text. Co-authored-by: shirruy <63145023+shirruy@users.noreply.github.com> * fix(deepseek): upload only images from roles DeepSeek accepts System and assistant message images were uploaded and persisted even though DeepSeek rejects images in those roles. Restrict uploads to user and developer messages plus tool outputs. Co-authored-by: fontanierh <14199823+fontanierh@users.noreply.github.com> * fix(openai-chat): make the first terminal flush the only bare-call recovery decision Content that arrives after finish_reason kept standalone recovery enabled, so a later cut-off block could still execute at EOF or [DONE]. …l-result handling for 2.90 (#7395) * fix(opengateway): normalize paired tool result inputs Carry the input-normalization slice of draft #7281 (16263b7). Preserve call/result identities and image content while projecting text arrays; reject unsupported or malformed parts explicitly instead of forwarding them. Keep the normalizer independently covered by mocked, immutable-input tests. Co-authored-by: HeiTuz <79418013+HeiTuz@users.noreply.github.com> * fix(opengateway): inspect empty completions with bounded replay Carry the independent completion guard and replay-safety tests from draft #7281 (16263b7). Inspect SSE/JSON only before meaningful output, release the held prefix on time/size expiry, and permit at most one eligible retry callback. Release the owned reader lock on cancellation and remove a redundant counter. Co-authored-by: HeiTuz <79418013+HeiTuz@users.noreply.github.com> * fix(opengateway): integrate completion recovery with the send budget Port the remaining adapter/dispatch integration from draft #7281 (16263b7) onto current dev. Map input incompatibility to HTTP 400 before any send; cap empty-completion recovery at one physical send, including HTTP 503 and connection-reset failures, while preserving existing admission and accounting. Cover the actual handler with mocked fetches and keep English/Korean guidance consistent. Split the OpenGateway structure contract to stay within document budgets and distinguish native recovery from the translated opt-in guard. Verification: typecheck, 118 focused tests, 27 layout tests, 9 size-ratchet tests, structure and privacy checks, and the docs build passed. Changed-mode rerun passed 19780 tests with 34 skips (30s timeout, two workers); the first run had one unrelated package-tree subprocess timeout, which also passed standalone. Co-authored-by: HeiTuz <79418013+HeiTuz@users.noreply.github.com> * fix(mistral): preserve cache keys and recover from rate limits Canonical key-auth Mistral Chat now sends prompt-cache keys, paces each credential/model pair from measured refusal headers with bounded, abortable waits outside the upstream header deadline, and retries 429s with a slower default budget. A server delay beyond the retry budget is kept non-replayable and its Retry-After reaches the caller. Relays, other adapters, OAuth credentials and xAI are unchanged. Co-authored-by: adaurban <31841049+adaurban@users.noreply.github.com> * fix(mistral): pace native Chat sends through the Mistral transport A single-key native Chat route never re-resolved its transport, so the new Mistral retry default replayed an over-budget Retry-After after 60 seconds instead of returning the refusal. Co-authored-by: adaurban <31841049+adaurban@users.noreply.github.com> * fix(opengateway): share the exact-URL boundary between normalization and recovery Co-authored-by: HeiTuz <79418013+HeiTuz@users.noreply.github.com> * test(responses): inventory the OpenGateway completion owners Co-authored-by: HeiTuz <79418013+HeiTuz@users.noreply.github.com> * fix(mistral): keep evicted pacing deadlines in force At 128 live identities the pacer dropped the oldest deadline, so that credential/model pair could send before its Retry-After. Remember the latest evicted deadline and make identities without an entry wait it out; memory stays bounded and the only cost is conservative pacing under extreme fan-out. Co-authored-by: adaurban <31841049+adaurban@users.noreply.github.com> * test(layout): keep B4b registrations within the layout file budget Merged with #7397, the three new rows pushed layout.json to 2001 lines. Fold them onto neighboring rows; the file stays at dev's 1998. Co-authored-by: adaurban <31841049+adaurban@users.noreply.github.com> Co-authored-by: HeiTuz <79418013+HeiTuz@users.noreply.github.com> * fix(mistral): hold only same-key replays on an over-budget Retry-After Marking the refusal non-replayable also blocked key-pool rotation and sanitized the upstream body. Use a same-target retry hold that only the same-key 429 loops check (native Chat, adapter dispatch and continuation, image and web-search loops), carried across response rewraps, so another pooled key can still serve and the original status, body and Retry-After reach the caller. Also keep an evicted identity's live deadline in force when it re-enters with a shorter delay. …he bypass-token cold-spawn disposition (#7408) * fix(codex): pin Windows shim refresh identities with open descriptors On Linux (ext4/tmpfs) a freed inode number is handed straight to the next file or symlink, and link(2) of a symlink source publishes the symlink itself. The refresh compared bare dev/ino, so a stage or aside swapped for a symlink or new file could carry the recorded numbers and be published or restored as the launcher. The stage and the moved wrapper are now held open for the whole transaction (an open inode cannot be reused) and identities include whether the entry is a symlink, so the guarantee no longer depends on platform link(2) or inode allocation. Fixes #7401 * test(ci): record the cold-spawn disposition for the shim bypass-token test Fixes #7400 * fix(codex): release the aside descriptor when its stat fails holdExisting now closes the descriptor on every unsuccessful path, including a throwing fstat, so the refresh hands the moved wrapper back instead of leaking the descriptor and leaving the launcher missing. Split CI aggregate matrix cases and SSH discovery scenarios into bounded asynchronous child invocations. Preserve gate-policy and argv assertions while allowing real candidate probes to finish and validating process cleanup. Co-authored-by: Claude Opus 5.5 <noreply@anthropic.com>
… block (#7455) * fix(kiro): read the whoami identity when kiro-cli appends its Profile block kiro-cli 2.32 prints a human-readable "Profile:" block on stdout after the `whoami --format json` object once an IAM Identity Center profile is selected. readKiroCliIdentity parsed the whole stream with JSON.parse, so the parse failed and the user email was silently dropped. Without that email, users of one organization profile collapse onto the same ARN identity (#6641): an import of a second user overwrote the first user's account, or, once that account had an email, was refused. Read only the leading top-level JSON object, which must end its line. Text on later lines is ignored; a second value on the same line still leaves the identity unread. * docs(kiro): describe the whoami identity contract of readKiroCliIdentity
Capture the bounded x-reasoning-included value from Bun's handshake event and publish it only after a successful open. Seed each canonical Responses exchange from its socket's validated default while preserving per-response metadata precedence and fallback behavior. Add real loopback and malformed-upgrade regression coverage, and document the HTTP/SSE scope and limits.

the surrounding documentation unchanged.
Review comments at @docs-site/src/content/docs/ko/guides/codex-integration.md:
- Line 222: Update the Korean “추론 사용량 표시 보존” qualifier to state “2.90.0 이상”
instead of “다음 릴리스 반영 예정,” matching the corresponding English documentation
update and keeping the locale pages consistent.
Review comments at @src/codex/routing/failure-window.ts:
- Around line 41-43: Remove the redundant isCodexAccountGenerationLive check
from noteCodexFailureWindowOutcome, leaving generation resolution and the
appendSample call intact; rely on appendSample’s
beginCodexAccountGenerationLiveCheck validation to reject stale generations.
Review comments at @src/oauth/chatgpt-shared.ts:
- Around line 49-58: Update the OAuth upsert email comparison and the non-Kiro

Review comments at @src/providers/mistral-transport.ts:
- Around line 104-107: Update MistralBudgetPacer.acquire so MAX_IDENTITIES
counts only requests that are about to wait, not immediately ready requests.
Move the queue-capacity check and increment into a guarded enter-wait helper
used before recovery sleeps and probe waits, and decrement this.waiting only
when that request was counted. Add a regression test beside the existing
queue-pressure test confirming 128 waiters for one identity do not block
acquire("other-identity").
Review comments at @src/providers/quota/failure-codes.ts:
- Around line 7-12: Remove the private quotaHttpFailure helper from the
Antigravity provider and import the shared httpQuotaFailure function from the

Remove the "pending next release" qualifier because this release ships the behavior.
Line 145 labels the x-reasoning-included relay as "pending next release". This PR promotes the candidate to stable 2.90.0. The PR also includes the implementation: readCodexWsUpgradeHeaders and CodexWsMetadata.seed in src/server/responses/codex-ws-metadata.ts:17-49,131-137, and the handshake capture in src/server/responses/codex-ws-session.ts:80-91.
After the merge, readers on 2.90.0 will read that the behavior is not yet available, but it is. Replace the qualifier with the version, or remove it.

This tells Codex that the server's reported usage already includes earlier reasoning. MetadataThe Korean page docs-site/src/content/docs/ko/guides/codex-integration.md:222 has the same qualifier. Update both pages together so the locales do not contradict each other.

Remove "다음 릴리스 반영 예정" (pending next release) to match the 2.90.0 release.
This is the Korean copy of the English qualifier at docs-site/src/content/docs/guides/codex-integration.md:145. The relay of the x-reasoning-included header ships in this release. After the merge, the qualifier is out of date. Change it in the same edit as the English page so the two locales stay consistent.

Remove the duplicated liveness read in noteCodexFailureWindowOutcome
Line 42 calls isCodexAccountGenerationLive(accountId, generation). Through readCodexAccountRecord, that call loads the whole codex-accounts.json store, including hardenConfigDir, hardenExistingSecret and notifyCodexStoreLoaded.
appendSample then loads the store again with beginCodexAccountGenerationLiveCheck() (Line 124). It also rejects the same stale generation (Line 127). Every tagged upstream outcome therefore pays for two full store reads.
There is a further cost on the read path. isCodexFailureWindowDegraded runs once per candidate account in failureWindowSteersNewThreads. When a window has tagged samples, each of those calls reaches recompute, which loads the store a third time (Line 67).
Line 42 adds no safety, because Line 127 performs the same check.

Normalize whitespace in email identity comparisons.
The lowercasing concern does not apply because the previous extractEmail implementation already lowercased top-level email claims. This change also trims them. The OAuth upsert and reauthentication paths lowercase both operands but do not trim the stored email, so a legacy credential containing surrounding whitespace can fail matching after re-login.
Use trim().toLowerCase() on both operands in these comparisons.

Make the process-wide queue_full limit apply only to requests that actually wait.
Line 106 compares one module-level counter, this.waiting, with MAX_IDENTITIES (128). It does this before it reads the caller's own deadline or recovery state. The module creates one MistralBudgetPacer at Line 157 and shares it across every Mistral credential and model.
How the failure happens:
- A refusal on credential A / model X sets a 60 s minute hold (
MINUTE_MS) or a long serverRetry-After. - Up to 128 concurrent requests for identity A park in
acquire, at theawait this.sleep(...)on Line 121 or behind the probe on Line 131. Each one holdswaiting++. - A request for credential B arrives. B has no deadline and no recovery state, so it could be sent right away. It still fails at Line 106 with
RequestPacingQueueOverloadError("mistral", "queue_full", 60). - The caller then answers HTTP 429 with
Retry-After: 60.
Result: one rate-limited key or model blocks every other healthy Mistral key and model in the process for up to a minute. On a shared proxy this is a cross-tenant availability failure, and it is reachable under ordinary load with a minute hold. Note that requests that do not wait cost nothing here: finally { this.waiting--; } runs in the same microtask. So only real waiters count, and that is exactly why one hot identity can fill the limit.
Fix: count a request against the limit only when it is about to wait. A per-identity limit is also fine.

Remove the leaked + diff marker in front of test.each.
Line 288 reads + test.each(["none", "identity", "bearer"] as const)(...). The leading + is a unified-diff marker that ended up in the source. JavaScript parses it as a unary plus on the return value of the test.each(...)(...) call. The test still registers, and the statement evaluates to NaN. So the suite does not break today. But the line is clearly unintended, it confuses readers, and a stricter lint rule (for example no-unused-expressions) would flag it.
