Summary

Ten intermittent or Windows-only test failures now wait on a real signal or no longer pay for unrelated cold-start work. Item #10 of the original list (restart-replacement-not-ready) landed separately in #7502, and this branch takes that version unchanged. No timeout was raised, nothing retries, and no assertion was dropped. Two items change product code: the PowerShell DPAPI helper (#2) and the update-restart home identity (#8). The rest are test-only.

# Test Cause Fix
1 claude-intercept-proxy pending async choice Bun.sleep(20) then settle(); when the proxy had not yet called selectTunnel, settle was unset (TypeError: settle is not a function) Resolve a promise inside selectTunnel and await it
2 desktop-compatibility-authority PowerShell helper (~50 s) The helper ran Add-Type and ConvertFrom-Json. The first cmdlet sends PowerShell through command discovery and module auto-loading, and scripts/test.ts gives every Windows run a fresh USERPROFILE, so there is never a module-analysis cache Product: the helper uses only .NET calls (Assembly.Load, String.Split, base64), and its stdin request becomes <operation>\\n<base64>. A new cross-platform test fails if a cmdlet comes back
3 gui-pair-http standalone CLI pairing (>48 s readiness) The first ocx start child loaded the whole server graph cold inside the readiness watchdog, with no access to the run's transpiler cache beforeAll warmModuleGraph on src/cli/index.ts (101 modules); pass BUN_RUNTIME_TRANSPILER_CACHE_PATH to the children
4 codex-prompt-lock stale-observation interleaving Parent existsSync then JSON.parse while child B was still writing .b Child writes .b.tmp, then renameSyncs it into place
5 cli-route-policy-write actual routing editor (20,038 ms on Windows, run 39646346553) The --eval child loads the management API, routing and Lab graphs cold inside its 20 s deadline Move the script to module scope; beforeAll warmModuleGraph on it (5/5 loaded); pass the transpiler cache to the child
6 responses-native-main-refresh 401 refresh and replay (17 s) Every case's fresh home took a spend-ledger lease and rewrote auth.json through a PowerShell SID lookup plus real icacls Synthetic principal plus stubbed sync and async icacls runners per case, the seams other suites already use
7 codex-catalog-write-audit owner-only create reported skipped A fresh audit file is hardened inside a 1 s product deadline, and a cold process spent it on the SID lookup and icacls Same seams in the file's shared beforeEach/afterEach
8 cli-update-restart-home same-path directory replacement did not throw Home identity compared ino as a JS number. An NTFS file ID carries the MFT sequence in its high bits, so above 2^53 a replacement directory with a neighbouring MFT index rounds to the same ino Product: identity adds an exact bigint fileId (dev:ino), and the child marker requires it. A stat seam lets a new test prove that IDs one apart above 2^53 are distinguished on any OS
9 desktop-compatibility-launch real COM validation (10 s ETIMEDOUT) The first run in a fresh profile pays PowerShell start, Add-Type module discovery, the C# compiler and COM activation inside the 10 s bound warmColdSpawn replays the same validation once in a Windows-only beforeAll; the 10 s bound now times a warm run
10 restart-replacement-not-ready Fixed on dev by #7502 (the same platform-correct assertion) Not in this diff
11 server-anthropic-billing returned 499 instead of 200 after 4.3 s The request runs under the test's own AbortSignal.timeout(2_000), and within that window three icacls calls harden a directory the request creates. Cold runners spent the 2 s on them, and the handler answered the caller abort with 499 Same ACL seams per case; the 2 s signal is unchanged

structure/clients/codex-desktop.md records the cmdlet-free helper. structure/ops/service-and-sidecars.md records the exact home fileId.

Verification

All local runs used isolated HOME, CLAUDE_CONFIG_DIR, OPENCODEX_HOME and CODEX_HOME on macOS (arm64). Each red run used origin/dev's file, or the pre-fix code, in a scratch copy:

Gates at the PR head:

  • bun run typecheck: exit 0
  • bun run structure:check: passed
  • bun run privacy:scan: passed
  • After merging dev (conflict only in the test(server): expect the platform's exit signal for a replacement that ignores SIGTERM #7502 file, resolved to dev's version), all 19 touched test files pass at the merge head: 452 pass, 5 skip (Windows-only), 0 fail.
  • Full suite: bun run test under /usr/bin/lockf -t 3600 /tmp/ocx-heavy-test.lock at 15c4043bbf exited 1. The parallel lane ran 50,806 tests in 2,537 files: 50,656 pass, 130 skip, 20 fail.
    • 14 failures are base-identical. Running the same files on the clean base 5740cf010b fails the same counts: Antigravity UA/wire/OAuth/fingerprint (8), desktop-compatibility-routing observation revocation (5), and claude-desktop-remote-hub cold restart (1).
    • 6 were a regression from [codex] Add Codex autostart ensure fallback #8: hand-built marker fixtures without fileId, fixed in 947460ee69. All 9 update-restart and system-restart files then pass 144/144.
    • The serial server-live and service lanes failed because the vp Bun 1.4.3 fallback got a registry 502 and then hung. With the cached Bun 1.4.3 binary they pass 55/55 and 212/212. All other serial lanes exited 0.
    • I did not repeat the full suite at the final head 947460ee69. The only change since 15c4043bbf is the fixture fix, which those 144 cases cover.

Not run locally: Windows. The real effect of #2, #3, #5, #6, #7, #9 and #11 on cold Windows runners, and Windows PowerShell 5.1 executing the rewritten DPAPI script, need a lane=all Cross-platform dispatch. This PR touches Windows-sensitive code and should not merge before that dispatch passes.

Checklist

  • Scope stays focused and avoids unrelated cleanup.
  • Docs or release notes were updated when needed.
  • Security-sensitive changes were reviewed for secrets, auth, and unsafe defaults.

📝 Walkthrough

The changes add exact directory identity to guarded CLI restart checks and revise the Windows DPAPI PowerShell helper’s input handling. Integration tests also add startup warmups, Windows ACL test setup, and explicit synchronization or platform-specific assertions.

Changes: Guarded CLI Restart Identity

Layer / File(s) Summary
Capture and validate exact directory identity
src/cli/update-restart-home.ts:1-47, src/cli/update-restart-child.ts:23-27
Restart-home capture records an exact fileId from bigint device and inode values, or from an injected stat callback. Marker parsing validates the digits:digits format.
Update identity fixtures, regression coverage, and documentation
tests/cli/cli-update-restart-home.test.ts:31-46, tests/cli/cli-update-restart-child.test.ts:9, tests/cli/cli-update-restart.test.ts:9-17, tests/cli/system-restart-client.test.ts:58, structure/ops/service-and-sidecars.md:61
Restart fixtures now include fileId. A regression test checks that incrementing an ID above 2^53 is detected even when the numeric conversions match. The documentation describes comparison using canonical paths and exact identity.

Windows DPAPI PowerShell Helper

Layer / File(s) Summary
Change DPAPI request parsing and verify the script
src/codex/desktop-compatibility/windows-key-protection.ts:13-30,136,173-177, tests/clients/desktop-compatibility-authority.test.ts:9-10,169-174, structure/clients/codex-desktop.md:72-79
The caller sends the operation and base64 payload on separate lines. The script loads .NET security support directly, requires two input lines, and compares operation names case-sensitively. A test checks for PowerShell cmdlet names, and documentation describes the helper and its test coverage.

Integration Test Reliability and Platform Setup

Layer / File(s) Summary
Warm child-process test commands before execution
tests/cli/cli-route-policy-write.test.ts:1-9,195,260-283, tests/gui/gui-pair-http.test.ts:1-221, tests/clients/desktop-compatibility-launch.test.ts:1-3,151-170
Routing-editor, GUI pairing, and Windows activation tests warm their module graph or validation command before execution. The tests pass an available transpiler-cache path to child processes.
Configure synthetic Windows ACL behavior in tests
tests/codex-integration/codex-catalog-write-audit.test.ts:32,42,83-102, tests/responses/responses-native-main-refresh.test.ts:27-28,46,78-83,124-126, tests/server/server-anthropic-billing.test.ts:15-16,39-47,66-68
These test suites configure a synthetic Windows principal and successful ACL runners before each test, then reset the hooks afterward.
Make asynchronous and platform assertions explicit
tests/claude-integration/claude-intercept-proxy.test.ts:183-198, tests/codex-integration/codex-prompt-lock.test.ts:252-255, tests/server/restart-replacement-not-ready.test.ts:46-56
The tunnel test waits for an explicit selector signal instead of sleeping. The lock test publishes completed output by renaming a temporary file. The restart test expects a platform-specific termination signal.

🎯 Functional Correctness | 🟡 Minor | ⚡ Quick win

Synchronize the child before asserting SIGKILL.

Line 52 sets a 300 ms deadline. The child installs its SIGTERM handler only after it starts. If the handler has not run by the deadline, the first SIGTERM ends the child, so this assertion fails instead of observing escalation.

Add a readiness handshake after handler registration, or do not require SIGKILL until readiness is established. This follows the provided terminateReplacement signal sequence.

Rebased onto dev bf4eb1209 as d7427e752 (merge manager). The restart-replacement commit (15c4043bb) was dropped: #7502 landed the same platform-correct assertion on dev first, so item 10's fix is already on dev. The other 11 commits are unchanged. A fresh Cross-platform CI lane=all on the new head is https://github.com/lidge-jun/opencodex/actions/runs/39720938104.

Merged with owner authority per maintainer decision 2026-10-21 (jun): the project owner authorized owner-path (--admin) merges for the pending green PRs after the 2.90.0 release; no second-maintainer approval is available.

Exact-head verification on d7427e7522ece216aa9a1d7f7be4b81810d82f2e: all checks completed with no failures, the PR is mergeable, there are no change requests, and no unresolved current Major/Critical/P0/P1 review threads. A local squash of this head onto current dev bf4eb1209 passed typecheck, structure:check, the file-size, import-cycle and test-layout ratchets, and this PR's own tests ( 471 pass 0 fail ). Windows-sensitive: true (Cross-platform CI lane=all on this head: https://github.com/lidge-jun/opencodex/actions/runs/39720938104); PR CI never runs Windows (AGENTS.md CI policy), so a Windows-sensitive PR merges only after a green lane=all dispatch. The merge is a squash pinned with --match-head-commit.
