📐 Maintainability & Code Quality | 🟡 Minor | ⚡ Quick winUpdate the family index description.
The agents page now documents planning, confirmed POSIX apply, and durable job status. The index still describes it as “read-only Codex CLI update inspection,” which is incomplete.

When parsed.json is enabled, also emit a machine-readable JSON error with the
job_not_found identifier before throwing.
Review comments at @src/codex/cli-update-apply.ts:
- Around line 180-185: Remove the per-archive verifyCodexCliArchive call in the

Review comments at @src/codex/cli-update-artifacts.ts:
- Line 9: Remove the duplicate CODEX_CLI_REGISTRY and isSha512Integrity
definitions from the plan module and import both from the existing exports in
cli-update-artifacts.ts. Re-export CODEX_CLI_REGISTRY from the plan module to
preserve its existing importers, and use the shared definitions for registry
arguments and integrity checks.
Review comments at @src/codex/cli-update-job.ts:
- Around line 286-293: Separate the `run()` failure handling from result
persistence in the job execution flow: use the synthetic ambiguous result only
when `run()` fails, and when it returns a result, preserve that result if
`finishCodexCliUpdateJob` fails. Do not replace a completed install’s outcome
with `job_result_unknown` during a persistence retry; keep the outer failure
path from inventing a different result.
- Line 289: Update runCodexCliUpdateJob to return a nullable
CodexCliUpdateJobDto because readCodexCliUpdateJob can return null when the job
is unavailable. Handle that result in both callers: have the CLI print a fixed
“job unavailable” message and exit nonzero, and have the HTTP route ignore the
null result.
- Line 248: Before the MAX_JOBS capacity check on store.jobs, remove the oldest
applied, not_applied, or refused jobs while retaining queued, running, and
ambiguous jobs; throw store_full only if the store remains full. Update the
--request-id retry guidance to note that removing a terminal job also removes
its request-ID deduplication.
- Around line 269-271: Update finishCodexCliUpdateJob to validate result.reason
and result.cleanupErrors before persisting: use SAFE_REASONS to replace unknown
reasons with a safe reason and ambiguous outcome, and retain only allowlisted
cleanup errors, capped at eight. Add tests covering current refusal and
apply-result reasons, plus unknown reason and cleanup-error values followed by a
store read.
- Around line 145-153: Update withLock to handle EEXIST through bounded,
non-blocking retries and report store_busy when the deadline expires; expose
that state as HTTP 503 with a Retry-After header. Ensure runCodexCliUpdateJob
preserves and persists the installer’s completion result when lock contention
occurs. Add a documented stale-lock recovery path that preserves the existing
owner-identity fencing, reports stale-lock state separately, and never reclaims
a lock based only on age.
Review comments at @src/codex/cli-update-lease.ts:
- Around line 40-42: Update acquireCodexCliApplyLease to return distinct
outcomes for an unsupported prefix and an already-held lease, while preserving
the acquired lease result. In the cli-update-apply flow, map those outcomes to
separate refusal reasons, and add the unsupported-prefix reason to the fixed
reason codes in cli-update-job.
Review comments at @src/codex/cli-update-plan.ts:
- Around line 562-563: Retarget the registry, isolation, and compile-cache tests
to exercise resolveCodexCliUpdateArtifactTarget, the resolver used by
createCodexCliUpdatePlan. Update their fixtures and expected query counts for
its JSON metadata queries, and cover its maxBuffer and platform-artifact
behavior; remove resolveCodexCliUpdateTarget only if no callers remain.
Review comments at @tests/cli/cli-codex-cli-update.test.ts:
- Around line 237-277: Extend the test covering “apply retry keys” to verify
that an accepted finish with an unsafe reason leaves the job readable, that
admission succeeds after 64 terminal records are removed, and that an existing
admission.lock does not replace a real applied result with job_result_unknown.
Exercise these cases through admitCodexCliUpdateJob, finishCodexCliUpdateJob,
and readCodexCliUpdateJob as appropriate.

Return a nonzero status when the job is unavailable.
runCliAction ignores callback return values and returns 0 whenever the callback resolves. The current return therefore makes the missing-job command succeed. Throw an error so runCliAction returns 1. Emit a JSON error when --json is used so callers can parse the response.

Verify each staged archive once after staging both archives.
The archives are verified twice, but reusing the first results is not safe. verifyCodexCliArchive checks archive contents, integrity, and path identity. A second pass can detect a staged-file change between the first pass and the later npm read. It does not make the later read atomic, but it provides a distinct check.
Remove the first pass and keep the later result-producing pass:

Keep one CODEX_CLI_REGISTRY and one isSha512Integrity.
cli-update-plan.ts repeats both definitions:
CODEX_CLI_REGISTRYat line 57 duplicates the constant at line 9 of this file.isSha512Integrityat line 135 duplicates the exported function at line 23 of this file.
The registry value gates two security decisions. This file uses its copy for the tarball origin check in officialCodexTarball (line 31). The plan module uses its copy for the --registry= arguments (lines 153 and 352) and for the origin comparison at line 444. The values match today. If only one copy changes, npm can query one registry while the origin check accepts another.
This file does not import the plan module. The plan module already imports from this file (line 10), so re-exporting the constant from here adds no new import edge.
Remove the local definitions in cli-update-plan.ts and import them from this file. Keep the constant re-exported so existing importers of the plan module still resolve.

Handle lock contention without losing job state.
withLock uses an exclusive mkdirSync call without retrying EEXIST. The CLI and HTTP paths use the same default job-store path, so cross-process contention is reachable. HTTP converts the failure to 503 admission_failed; CLI admission throws before the job starts.
Contention can also lose the completion result. runCodexCliUpdateJob calls finishCodexCliUpdateJob after the installer completes. If the lock is busy, its catch path calls finishCodexCliUpdateJob once more. If the lock remains busy, that failure escapes and the real result is not persisted.
The crash behavior is deliberately fail-closed. A lock left after a process dies is not reclaimed by this code, and future mutating operations remain blocked. Status reads still work, so “every caller” is too broad. The inspected CLI contract exposes apply and status, but no lock-recovery operation.
Add a bounded contention path that avoids blocking the server event loop, reports store_busy after the deadline, and returns Retry-After from HTTP. Do not delete a lock based only on age. Add and document a recovery path that preserves the existing owner-identity fencing, and report stale-lock state separately.

After 64 jobs the store is permanently full, because no record is ever removed.
How it fails:
- Line 248 throws
store_fulloncestore.jobs.length >= MAX_JOBS(64). - Nothing in this module removes a record. That includes terminal
refused,not_appliedandappliedjobs. - The CLI parser (
src/cli/codex-cli-update.tsLine 179) creates a newrandomUUID()request ID when--request-idis omitted. Every plainapply --plan-id … --yestherefore adds a new record. - Refusals are a normal result (
plan_changed,already_current,blocked_active_session, and others), so records build up quickly.
Result: once 64 records exist, every CLI apply fails. HTTP apply returns 503 store_full. No supported command clears the store.
The docs only promise to keep unresolved records ("Unresolved records are retained rather than expired"). Terminal records can be removed safely.
Fix: under the same lock, before the capacity check, remove the oldest records whose status is applied, not_applied or refused. Keep queued, running and ambiguous records. Fail with store_full only if no terminal record can be removed.

A failure while saving the result replaces a completed install's real outcome with ambiguous/job_result_unknown.
How it fails:
run()(Line 286) andfinishCodexCliUpdateJob(Line 287) share onetry.- If
finishCodexCliUpdateJobthrows afterrun()returned a realappliedresult (lock busy,ENOSPC,unsafe_store), thecatchbuilds a syntheticambiguousresult withleaseRetained: trueand callsfinishCodexCliUpdateJobagain (Line 293). - If that second call succeeds (the lock was released in the meantime), the store records
ambiguous/job_result_unknownwithleaseState: "unknown". The verifiedappliedresult is gone. - If the second call also throws, the promise rejects. The
finallyremovesliveandstarted, and the job staysrunningforever. Readers then seeowner_state_unknown.
The operator gets an ambiguous record for an install that was verified. They then have to investigate a job that actually succeeded.
Fix: keep run() failures separate from failures that happen while saving the result. If run() returned a result, retry saving that same result and never substitute the synthetic ambiguous one.

Separate the `run()` failure handling from result persistence in the job
execution flow: use the synthetic ambiguous result only when `run()` fails, and
when it returns a result, preserve that result if `finishCodexCliUpdateJob`

readCodexCliUpdateJob(...)! can return null here, and the CLI then crashes with a TypeError.
runCodexCliUpdateJob declares Promise<CodexCliUpdateJobDto>. On this branch (and at Lines 287 and 293), readCodexCliUpdateJob returns null when the job record is missing. That happens when the run-start lock callback throws job_generation_changed because index < 0. The ! only hides the null from the type checker.
src/cli/codex-cli-update.ts Line 328 then reads job.jobId and throws an unhandled TypeError instead of a clear message.
Fix: change the return type to Promise<CodexCliUpdateJobDto | null>. Then handle null in both callers: the CLI prints a fixed "job unavailable" message with a nonzero exit, and the HTTP route ignores it.

Update runCodexCliUpdateJob to return a nullable CodexCliUpdateJobDto because

🎯 Functional Correctness | 🟡 Minor | ⚡ Quick win
An unsupported prefix returns the same null as a held lock, so apply reports installation_busy.
acquireCodexCliApplyLease returns null from lines 41-42 in these cases:
- the prefix is not a canonical directory,
- the prefix is group- or world-writable,
- the prefix is not owned by
process.geteuid().
It also returns null when mkdirSync fails with EEXIST because another apply holds the lock. src/codex/cli-update-apply.ts line 161 maps every null to refused / installation_busy.
Example trigger: a Linux npm-global prefix under /usr/local owned by root, with apply run as a normal user. Another example is a group-writable prefix. The operator sees "busy", waits, and retries. The result never changes, because the cause is a permanent prefix-policy refusal and not contention. The durable job record keeps the misleading reason.
Return a discriminated result and map each case to its own reason.

Retarget resolver tests to the production resolver.
createCodexCliUpdatePlan uses resolveCodexCliUpdateArtifactTarget, but the registry, isolation, and compile-cache tests call resolveCodexCliUpdateTarget. They do not cover the production resolver's --json metadata queries, maxBuffer setting, or platform-artifact resolution.
Update those tests to call resolveCodexCliUpdateArtifactTarget and adjust their fixtures for its JSON responses and query count. Remove resolveCodexCliUpdateTarget if no remaining caller requires it.

Add tests for the failure modes the store does not handle yet.
The suite covers deduplication, stale writes, unsafe ancestry and lifecycle projection. It does not cover three cases that break the store in practice:
- An apply result whose
reasonis not inSAFE_REASONS(for example"late_result") goes through the accepted finish path. The test should check that a laterreadCodexCliUpdateJobstill works. Today the stale-write test at Lines 271-275 passes only because the fenced write is rejected first, so the corrupting write never happens. - Admitting 65 jobs that end in a terminal state. The test should check that the 65th admission succeeds once terminal records are removed.
- An existing
admission.lockdirectory whilefinishCodexCliUpdateJobruns. The test should check that the realappliedresult survives and is not replaced byjob_result_unknown.
Each test maps to one comment in src/codex/cli-update-job.ts and fixes the expected behavior for its fix.
