v0.6 docs: persistent live sessions in CHANGELOG, README and design doc - #209

The changelog and README had no entry for the core v0.6 feature: live sessions that detach when a window closes, reattach through the startup prompt, /resume or nmsh --attach, list with nmsh --sessions, and keep output produced while detached. Add those entries, the Ctrl+Z fix from #191, and a design-doc section on the live-session lifecycle (#126-#130). Correct 'nmsh --list' to the real flag, 'nmsh --sessions'.

Why
The release-readiness audit found that the v0.6 CHANGELOG and README had no entry for the core v0.6 feature, persistent live sessions (#126–#130). Only #131's hardening and later features were listed. The #174 entries also named a flag that doesn't exist (
nmsh --list; the real one isnmsh --sessions).Changes
