Remove source tags from references

Summary

• remove source attribution tags from the reference Markdown files
• remove the visible Sources attribution line
• preserve prose, code, and unrelated comments

Validation

• independently audited 370 source-tag deletions across 25 files
• confirmed no source tags remain
• confirmed the Markdown word diff adds no tokens
• git diff --check passes
