…+ operation lookup + rules) PathItem parses the 3.2 query operation and additionalOperations map. In 3.2 documents (or with allow_3_2_features), operation()/request_operation() resolve them and match methods case-insensitively; earlier documents keep the exact-case standard-method lookup.
