Summary

Consolidate review records into one concise procedure with an explicit security-sensitive path, without changing merge gates.

Changes

• Replace duplicated advisory machinery with a compact routine-change procedure and examples.
• Preserve current-head provenance, finding disposition, conversation resolution, all five required contexts and explicit maintainer authorization for high-risk changes.
• Remove empty verification tables and routine checklist prompts from the PR template; retain breaking-change migration guidance.
• AGENTS and the playbook already link to this policy and remain consistent. No workflow, bot or required check is added.

Validation

• Python 3.11.15 make check-all and make build passed: 952 passed, 4 skipped, 4 deselected, 13 warnings in 11.61s; coverage 96.40%.
• Reviewed the policy against AGENTS, playbook and template. No live Azure or docs-site build was run.
• Policy change authorized by the maintainer's explicit instruction; final-head required CI and resolved conversations remain conditions of merge.

Reviewed exact head 7e2af86: all five required contexts are completed/success, branch CLEAN, no unresolved review conversations. The procedure preserves exact-head provenance, security-sensitive authorization, all required CI and conversation resolution; removes only duplicated advisory ceremony. AGENTS/playbook retain a single policy reference. Merge authorized by the maintainer’s explicit instruction.
