Record inline CHECK constraints for listed columns.
If ALTER TABLE t ADD (a INT CHECK (a > 0)) succeeds, record_add_column does not persist the inline CHECK metadata. The separate tableConstraintDef pass does not process a check nested in columnDefinition. Table recreation reads CHECK clauses from check_constraints, so it can omit this constraint and accept a = 0.
This limitation also existed in the pre-existing single-column ADD COLUMN path because both paths call record_add_column. The PR did not introduce the limitation, but the ADD-list path still needs the same inline CHECK handling as record_create_table.

I created #562 to track the inline CHECK gaps, including the existing single-column ADD case. I'll keep this PR focused on parenthesized ADD lists.

@JanJakes Understood. The inline CHECK gap predates this PR, so tracking it in #562 is reasonable. I won’t request that fix in this PR; keep this change focused on parenthesized ADD lists.

"ALTER TABLE t ADD [COLUMN] (a INT, b INT)" failed with a TypeError. The column name in a "columnDefinition" node is a "fieldIdentifier", not an "identifier", so the name lookup returned null. The list can also hold indexes and constraints, e.g. "ADD (a INT, INDEX (a))". These were skipped when the list had a column, and only the first one was recorded when it had none. Record all list items, columns first, like CREATE TABLE does.

Fixes
ALTER TABLE ... ADD (...)andADD COLUMN (...), which previously crashed when adding columns:These lists could also silently lose indexes and table-level constraints. The driver now processes every list item, adding columns before indexes and constraints, so an index can reference a column declared later in the list.
