/api/codex-auth/accounts?refresh=1 to qualify HTTP 401 handling: known terminal
authentication codes always produce needsReauth, while unrecognized 401

Independent isolated execution at b2a8fb30cdaf8715e4a23b2a5c9aeb458df48688 passed 12 headless account-listing tests. Actual Cross-platform CI, Catalog contracts and React Doctor have now run successfully. I checked the narrowed read boundary: only CODEX_HOME_UNAVAILABLE becomes an unchecked observation; listing does not create the native home or clear cached identity/reauth state, while credential claims remain strict and invalid explicit paths/unsafe storage fail closed. No live credentials or account refresh were used.
The unresolved documentation qualification is valid: structure/providers/openai-accounts.md:254 must distinguish a known terminal 401 (terminal regardless of token liveness) from an unrecognized 401 (marks reauth only when access is not verifiably live). Please apply that qualification rather than describing every 401 as terminal. The read-boundary result does not resolve that thread on your behalf.

Checked b2a8fb30cdaf...09e092bf6d5d: the only follow-up change is the one contract sentence requested in my review, and it now correctly distinguishes known terminal 401 codes from unrecognized 401s with a non-live token. That resolves my documentation concern; the runtime/authentication code is unchanged from my earlier 12 passing independent cases and manual read-boundary review. I approved execution for the new head after checking the one-line delta. Old-head CI and the author's 378-test receipt are not substituted for completed exact-head CI/readiness; those new-head checks still need to settle before final approval.

canonicalExistingDirectoryconverts non-directory and inaccessible paths intoCODEX_HOME_UNAVAILABLE. The account-list path catches that code atsrc/codex/auth-api/account-list.ts:443-446and returns the cached projection. Therefore, an explicit file atCODEX_HOMEcan make account listing succeed as if the home were absent, instead of rejecting the invalid configuration. Re-ran the existing regression on this head: bun scripts/test.ts tests/codex-integration/codex-auth-headless-accounts.test.ts -t 'an explicit non-directory CODEX_HOME'Result: 1 passed, 0 failed, 3 assertions. It asserts that listing rejects, the pool credential is unchanged, and the invalid-home file is unchanged. The broader current-head focused run passed 436 tests, and all four Ubuntu CI shards passed.
