Summary

Absorbs four community fixes to combo failover. Each was verified against current dev with a red-first regression and carried as the original author's commits. Three needed a small follow-up fix, each in its own commit with its own failing-first test.

Source Author Problem Disposition
#7317 @vadymhimself A zero-output response.incomplete with no stated reason was accepted as the final answer, so the combo never tried its next target. Absorbed. Follow-up 55a51b87f1 keeps a reasonless terminal snapshot that already carries text or tool output instead of replaying it on another provider.
#7329 @vadymhimself When target 1 failed after a 200 SSE, its relay's unbind settled the parent turn lease and target 2 was aborted ("turn already settled"), so the client got a 502. Absorbed. Follow-up a86f24b39f unbinds only a lease that actually registered a controller, which avoids an unmatched unbind on plain admission leases.
#7339 @luvs01 Combo signature/thinking-signature recovery rebuilds bypassed the request-wide shared send budget (8 physical sends against a ceiling of 7 in the fixture). Absorbed as-is (all three commits).
#7320 @vadymhimself A combo hop warning said only failed with 401, with no reason. Absorbed. Follow-up afc1d9dd01 redacts parsed fields before collapsing whitespace, so multiline credential framing in an upstream code cannot survive redaction.

The structure note on combo streaming boundaries moved to structure/transports/responses-combo-boundaries.md, because responses-failover.md would otherwise exceed its 600-line budget. The docs-site combos guide describes the new incomplete-retry rule.

This is the companion to #7354 (issue #7298). These four PRs were listed for that issue, but they fix separate failover defects, so they ship here as one bundle and keep the #7298 change reviewable on its own.

Checklist

Summary by CodeRabbit

  • Bug Fixes
    • Improved combo failover for zero-output incomplete responses while preserving terminal responses that already contain output.
    • Refined retry and recovery accounting so sends are counted when dispatched, with unsent attempts remaining uncharged.
    • Kept turn admission active through fallback execution and stream completion, cancellation, or caller abort.
    • Made combo failure warnings more useful while redacting credentials and excluding unparsed response bodies.
  • Documentation
    • Added guidance on combo streaming boundaries, failover, and retry behavior.

vadymhimself and others added 12 commits October 19, 2026 15:14
A combo target that ends with `response.incomplete` before any output and
without `incomplete_details.reason` was accepted as the turn's answer, so
the caller got an empty incomplete turn instead of failover to the next
target. Treat a missing reason like the transport reasons already listed;
semantic reasons such as max_output_tokens still commit the target.

Co-Authored-By: Claude Code <noreply@anthropic.com>
Each combo child relay binds the turn admission lease to its own controller
and unbinds it when its stream ends; the lease releases once nothing is
bound. When target 1 returned 200 SSE and then failed before output, the
combo cancelled that relay, its unbind released the parent's lease, and
target 2 was aborted on bind with "turn already settled" (502).

handleComboResponses now binds its own controller for the whole ladder and
unbinds it on return. The accepted child's stream is bound before that, so
it keeps the lease until it ends.

Co-Authored-By: Claude Code <noreply@anthropic.com>
The combo hop warning printed only `<target> failed with 401 after Nms`,
so a revoked credential or plan downgrade logged hundreds of identical
lines that read like a provider outage. Append the upstream error code and
parsed message (falling back to the normalized body text), collapsed to one
line, redacted and capped at 200 characters via sanitizeLogMetadataString.

Co-Authored-By: Claude Code <noreply@anthropic.com>
…ilure

Drop the classificationText fallback so an unparsed upstream body never
reaches the warning line; the log boundary is now the two parsed fields.

Co-Authored-By: Claude Code <noreply@anthropic.com>
Preserve credential framing until the shared redactor has removed credential values. Cover multipart framing in a parsed upstream code at the actual combo hop warning boundary.

Co-authored-by: Vadym O <bolein95@gmail.com>
A terminal-only upstream can include text or tool items in its final snapshot. Keep that response committed, and cover explicit semantic/unknown reasons and output-first streams as well.

Co-authored-by: Vadym O <bolein95@gmail.com>
Match registerTurn capability checks for plain admission leases so the ladder does not report a spurious release miss. Add accepted fallback stream completion, cancellation and caller-abort coverage with exactly-once attached lease settlement.

📝 Walkthrough

Combo streaming failover now classifies additional zero-output incomplete terminals for retry. Recovery sends share physical-send accounting, combo execution binds to turn leases, and exhausted-hop warnings can include sanitized upstream error details. Tests and transport documentation cover these behaviors.

Changes: Combo streaming failover and recovery

Layer / File(s) Summary
Stream preflight and retry boundaries
src/server/responses/combo-stream-preflight.ts, tests/routing/combo-stream-preflight.test.ts, docs-site/src/content/docs/guides/combos.md, structure/transports/responses-combo-boundaries.md, structure/transports/responses-failover.md, structure/INDEX.md, structure/manifest.json
A reasonless response.incomplete is retryable when it has no output items. A nonempty output snapshot remains accepted, and explicit non-retryable reasons remain terminal. The docs describe preflight, replay, and HTTP 410 boundaries.
Physical-send accounting and recovery
src/server/responses/adapter-dispatch.ts, tests/adapters/anthropic/anthropic-combo-account-cooldown.test.ts, tests/responses/responses-anthropic-thinking-signature-recovery.test.ts, structure/transports/anthropic-thinking-recovery.md, structure/transports/responses-spend.md
Combo and recovery dispatches use shared send allowances and physical-send reporting. Anthropic rotation reserves and settles recovery permits. Tests cover send counts, pacing refusals, and refunds.
Turn lease and failure logging
src/server/responses/core.ts, src/server/responses/core-combo.ts, tests/server/server-combo-zero-output-failover.test.ts, tests/responses/responses-combo-exhausted-error.test.ts, structure/transports/responses-combo-boundaries.md
Combo execution registers and unregisters its ladder abort controller with the turn lease. Hop warnings append bounded, sanitized upstream error details when available. Tests cover lease lifetime and warning redaction.

Combo failover warnings now include the upstream provider's error message. Some providers echo request content or account details in those messages, which this project's privacy rules forbid writing to logs. Log only the status and an allowlisted error code before merging. The retry, lease, and send-budget changes otherwise look ready.

Do not log the unallowlisted provider message.

failure.upstreamMessage can contain arbitrary provider-supplied error.message text. Secret redaction and length limits do not exclude request content or account identifiers. The repository privacy rule forbids logging those values. Keep the status and an explicitly allowlisted code, or use a fixed diagnostic.

P1 Badge Stop logging arbitrary upstream error messages

When a provider echoes submitted prompt text, tool arguments, or account details in a structured error.message, this appends that attacker/provider-controlled content directly to the server log. redactSecretString only masks recognized credential patterns, while whitespace normalization and the 200-character cap do not prevent disclosure of ordinary request content or identifiers. Log a fixed classification or an allowlisted code instead of the raw upstream message.
